MISC and COVID in Kids.

I feel so so grateful to have had the opportunity to chat with @lynzyandco on her new, incredible podcast @motherhoodmeetsmedicine !

We talked mothering and a LOT of practical MISC information because as a pediatric critical care doctor we are sadly still seeing a steady wave of MISC (multi system inflammatory disease in children).

Let’s talk for a second why we care about MISC.
1. Your kiddo will most likely end up on the ICU with a doctor like me. As much as I love my patients my goal is always to keep you safe and OUT of the ICU.
2. Sadly many of these children end up with invasive central and arterial lines, and in the very sick cases the end up with a breathing tube on the ventilator.

Now the good:
MISC is still a rare disease.

As of Feb 18th 3.1 million children have tested positive for COVID19- but only 2060 cases of MISC have been reported. However, OF those MISC CASES sadly 30 children have died.

More good news: We have treatments for MISC - several stages of treatments actually - and they currently are working very well!

One big takeaway message of the podcast: even if your child was never diagnosed with COVID19 they CAN STILL HAVE MISC. in fact, in my experience most parents say their child never ever manifested symptoms of COVID19.

Signs to call your doctor:
1) persistent fever (esp greater than 5 days
2) new rashes
3) GI symptoms: nausea, vomiting, diarrhea, ascites (your child’s belly suddenly looks bigger than normal)
4) conjunctivitis (your kiddos eyes look very red and swollen)
5) large lymph nodes (feel like balls underneath your child’s skin)
6) chest pain
7) confusion/delirium

It’s important to catch MISC early because the treatments appear to be more effective the earlier we administer them AND catching it early could prevent a trip to the ICU!

COVID SHOT and BREASTFEEDING!

Very very very very very exciting update for those of us who are breastfeeding and received the covid shot - it appears that maternal antibodies are, in fact, passed along to babies through mammas breast milk!

This is NOT surprising.
This is honestly just a confirmation of what we assumed would happen (if you don’t believe me go back to my posts about the covid shot and breastfeeding for proof!).

This is how our bodies work:
1. Mom gets exposed to a virus (in this case rather than the whole virus it’s the spike protein that we all know and hate that is associated with the SARS cov2 virus)
2. Mom makes antibodies toward that virus (in this case towards the spike protein)
3. Mom passes these antibodies to baby though breast milk (IgA, IgG are generally the most common)
4. Baby gets PASSIVE immunity to that virus

The body is amazing, and in many cases predictable. Thankfully, with regard to the covid shot, are bodies appear to follow a predictable course when it comes to antibody production and passage through breast milk.

Some caveats: this study only included 7 patients. With such a small sample we cannot form any conclusions. Furthermore - this study is a preprint, meaning it has not gone through peer review, which gives us even more reason to not jump to conclusions before this has happened.

Having said all that I, for one, am still very excited.

This day has never meant more that it does on this very day because of one simple reason: Sita.

I must teach her to be:
Strong
Resilient
Intelligent
Wise
Funny
And most of all - unflappable.

My hope is that when Sita is ready to enter the world she won’t face the same gender inequity that I have faced - that all of us have faced. But if she does have to experience it, if we haven’t done our job to fix the wrongs that our society was built upon - I want her to be prepared.

I want her to be fierce.
And I know she will be- because she already is.

Raising Sita has been the greatest joy of my life. Despite my hectic schedule as a #doctormom – I wouldn’t change a thing!

#sunbeamambassador One daily reminder I have of Sita’s birth is persistent back pain from my unexpected c-section. I have worked hard to recuperate my body safely and steadily – but the back pain still comes back from time to time. As a physician, I know heat therapy can greatly help ease muscular pain, but having the time to sit in one place while playing with my daughter or working feels impossible.

Cue the @sunbeam_us GoHeat™ Cordless  Heating Pad. As you can see, I just strap it to my lower back and go about my day. I'm embarrassed to say that I don’t always follow my own advice for rest and recuperation – they always say doctors make the worst patients – and I am no exception. As if being a parent isn’t hard enough, we pile on all this extra pressure to commemorate every step of their lives in the absolute perfect way.

I’ve been thinking about this a lot as Sita’s one year birthday is coming up in less than 2 short weeks. Thoughts like-

How do I commemorate her very first birthday?

Omg I’m not done with her baby book

What is Sita going to wear for her birthday?

What am I going to wear??

Who should I invite? Just family? Drive by birthday?

Oh sh$r I haven’t done our wedding album.

And so many more. Birthdays have a way of bringing out insecurities - as a person and a mother and I have been beating myself up for “not doing enough.” As If breastfeeding my child, Working more than a full time job, doing grant funded research, being a wife, dog and cat mamma, and maintaining our house isn’t “enough.”

But the thing is - I am enough.
Sita is loved.
My husband is loved, and he loves me.
And I love my job - it fulfills me.

The rest is all extra.

One thing I have taken care of (or started to take care of!) is Sita’s baby book. I decided to use @theshortyearsbooks after my close friend @amitakundramd recommend it. CHILDREN AND C0VID19.

I was planning on a happy post today about making a baby book. After the heaviness of the past few days I needed a moment of levity. However then this article popped up in my feed by an author I have long respected, @profemilyoster , a fellow women in academia.

Let me be clear - this post is not to tear Professor Oster down or “cancel” her. She has done so much for women - and I know she was well meaning in this article, but it sadly missed the mark.

Some truths the article highlighted (I like to start with the good):
1. kids are much less likely to get severe Covid
2. children appear to not be transmitting Covid in school —> important caveat: this is because they are wearing masks, practicing social distancing  and good hand hygiene
3. kids will likely  not get the shot until next winter (I hope this is wrong, but I suspect it isn’t)
Some issues I had with the article:
1. I don’t like blanket statements when it comes to health information - saying all kids are like vaccinated grandparents is irresponsible. This doesn’t take into account children with underlying health issues that are, in fact, at risk for acute covid
2. She relates that MISC (a consequence if COVID) is rare. She is right. But when it’s your previously healthy child in an ICU with central lines and medications to help their blood pressure - the rarity doesn’t seem to matter. I also need to mention that MISC requires experience and expanded laboratory capacity to diagnose. I mention this because I suspect that actual number of MISC cases is higher than what is reported by the AAP (see past post for details)

Ultimately, I understand why she wrote this article. She understands the pain and anguish that parents are going through with their children stuck at home and wants to help support them in making plans. But the sad truth is that most people will read that Atlantic headline and miss the nuances that she reports in the text of the article which I found less inflammatory and more moderate and data focused.

Talk to your doctors guys. We have so much hope for our future, let’s not go crazy and send us all back into a lockdown before we reach the finish line!

3800 REPORTED hate crimes in the past year.

I highlight “reported” because there are undoubtedly far more crimes that have remained silent or misclassified.

The onus is NOT on our AAPI colleagues to educate us, but sadly this is what is happening time and time again. The work needs to be done not just on social media, but in our own home, in our own communities. But I do fully believe using our own platforms on social media to push these realities to the forefront can be enormously powerful as well.

I stand with you - as a fellow AAPI member, as a human being.
